Go to view menu and then show layer transition controls, they are hidden by default.
You will now see the previous/next clip arrows for column triggers and for each layer appear to the left of your first column. You can now assign midi as normal.

You should be good with DVI-D to HDMI plus an active fiber HDMI Cable, but there's a chance the fibre could introduce a small lag (important if spanning/blending content), use the same on all outputs if possible. After playing a bit more, I see that upon making certain changes (such as adding layers) the highlighting works as expected when I save, but when I'm only making changes to clips it does not.
