I lost my .mp4 file icon above my video thumbnail movie files. looks like Alley is picking it up though. I was wondering how to get this. In the folder options the display file icon on thumbnails is on and in k-lite codec same option for mp4 is checked

This just looks like you have changed the file association for that file type to another program. See my screenshot where I have changed the association for .mp4 to Alley:
.mp4 set to Alley, .mov set to VLC
.mp4 set to Alley, .mov set to VLC

You can easily change this back by right clicking and picking open with->choose other then selecting the always open with this app/program checkbox.
